Multisystem Care Teams

Multisystem Care Teams are designed to help Franklin County families with youth under the age of 22 (0-21) who need a coordinated plan to address their unique needs.   The Multisystem Team Facilitator works to engage the family to create a team of people most important to them.  The team develops a family plan of care that includes creative strategies designed to address the family needs and build upon the family strengths. Teams meet regularly to evaluate the agreed upon strategies and make adjustments when needed.  The team celebrates successes with the family! 
Caregivers have the right to:

• Decide who should be part of the team

• Initiate a team meeting

• Be heard in team meetings

• Bring an advocate to team meetings

• Select or decline service options

Referrals can be made to FCFC by partner agencies or families.
